What you need:
- High school diploma or equivalent
- Individuals in this position must have obtained age twenty-one (21) prior to the first day of employment or transfer into the position.
- Current & valid PA licensure as a Licensed Practical Nurse (LPN)
- Current Cardiopulmonary Resuscitation (CPR) certification and First Aid Training
- Ability to read, write, and speak the English language fluently.
- Ability to reposition and turn residents and assist with transportation of residents within the facility.
- Ability to lift up to fifty (50) pounds occasionally and to perform the functions necessary to push a wheelchair.
- Excellent written and verbal communication skills and the ability to train end users.
- Proficient in the use of Microsoft Products such as Word, Excel and Power Point.
- Must be able to work with minimal supervision.
- Demonstrated ability to maintain a therapeutic attitude toward and effective relationship with the physically ill or infirm residents.
- Fit testing as needed per Luthercare’s Respiratory Program
- COVID-19 vaccination or able to obtain an exemption
What you do:
- Supervises an assigned resident personal care area and assigns work as appropriate. Provides instruction and direction in the methods and techniques of nursing and resident care. Ensures appropriate staffing coverage. Performs those functions required to manage staff including hiring, problem resolution, evaluations, disciplinary action.
- Develops and presents orientation and training programs to promote necessary skills of providers and ensures appropriate competencies are maintained. Observes staff performance in completing daily tasks and provides direction in improving the way work is completed.
- Assists with the prescreening of residents prior to admission and/or prior to return from hospitalization or other health care provider to determine appropriateness for personal care. Assists with the admission process.
- Demonstrates and coordinates accepted standards of performance by working as a Personal Care LPN and providing hands-on assistance to team members.
- Performs daily rounds to review documentation. Reports changes in resident condition to the Personal Care Manager/Administrator. Completes all necessary documentation. Provides communication to family members regarding important issues and updates. Ensures labs are completed as ordered and addressed.
- Assists the Personal Care Manager/Administrator with the inspection process in accordance with state regulations and the standards of Luthercare.
- Maintains current knowledge of billing, coding and insurance forms used in Personal Care.
- Assists with facility oversight in the absence of the Personal Care Manager/Administrator or at designated times.
- Provides communication with physicians regarding resident issues, orders, and treatments.
- Coordinates transportation with resident family members for appointments & provides Documentation of Medical Evaluation (DME).
- Completes Resident Assessment Support Plans (RASP) for residents in a timely manner, and other documentation required by the Department of Human Services (DHS). Contacts family members to review RASP after completion.
- Completes assessments on residents, including admission, discharge, and level of living.
- Completes rounds with physicians, as needed. Provides information/updates to nursing staff, supervisors, physicians, and other disciplines as appropriate to coordinate continuity of care. Attends clinical meetings as assigned. Assist with orders on units.
- May assist with daily care of residents, as needed, to include duties such as: administer medications, providing treatments as ordered by physician, and routine tasks such as BP, weights, intake/output, answering call bells, hands-on care.
- Adheres to all Personal Care standards, federal/state regulations and follows Luthercare’s infection control, fall prevention, general safety protocols and quality assurance and improvement standards.
