Overview:
Education: Graduate of an accredited school of Licensed Vocational Nursing or Licensed Practical Nursing. May be required to complete HHS’ Computer Based Training for appropriate category(ies) of license.
Licensure: Current license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of residence and/or in accordance with the Nurse Licensure Compact (NLC), current Driver’s License.
Experience: Minimum one year experience as a Licensed Nurse (LVN or LPN) obtained within the last 36 months. One year experience in home health care preferred. Management experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively with all segments of the staff. Demonstrates proven decision-making skills. Proof of current CPR.
Transportation: Reliable transportation. Valid and current auto liability insurance.

Responsibilities:
The Clinical Care Manager - LN serves as the liaison between referral sources, patients, caregivers, and employees. This role is critical to the coordination of care process and must have strong verbal and written communication skills. Serves as a regulatory and policy resource to other staff members. Oversees coordination of care among the healthcare team members, including contractors.
Participates in care coordination with agency staff and contractors.- Complete workflow tasks, Administrative Tasks, and other assignments to the Clinical Care Manager Responsible Position in the Agency’s electronic medical record.
- Receives and enters orders from all relevant physicians, Nurse Practitioners, and/or Physician Assistants involved into the plan of care and ensures the orders are approved by the responsible physician.
- Ensures effective communication with all physicians involved in a patient’s plan of care and other health care practitioners (as appropriate).
- Assist in the entry of patient referrals, as needed.
- Follows up on pending referrals to ensure timely patient admission in collaboration with the Intake Coordinator.
- Prepares for and participates in case conference in collaboration with other healthcare team members and participates in decision-making regarding patient recertification/discharge.
- Reviews patient occurrence reports and takes appropriate action.
- Reviews patient infection control reports to ensure appropriate action has been taken and items are reported to the QAPI committee.
- Reviews OnCall coordination notes and take appropriate action.
- Performs in-home visits as assigned.
- Monitors systems and identifies problem areas to Branch Director both verbally and through written reports.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Participates in Agency-sponsored in-service training.
