About the Organization and Position

Well known Non-Profit Health Agency is hiring for a Clinical Care Lead Case Manager.

This wonderful non-profit serves people age 60 and older, adults living with disabilities, and their families and caregivers in Lynn, Lynnfield, Nahant, Saugus, and Swampscott, Massachusetts for more than 45 years. They provide a continuum of community health and social services to help people maintain their independence, safely and with dignity. Providing a broad range of services including home care services; nutrition programs; transportation assistance; housing supports; clinical and protective services etc. The agency recently received a second prestigious 3-year accreditation for excellence in case management and care coordination from the National Committee for Quality Assurance (NCQA).

The NRLP Program Clinical Lead will be responsible for providing supervision and guidance to Program Coordinators (CPCs). The Clinical Care Lead will complete Assessments on all enrollees as well as provide oversight to enrollees’ discharge plans from the hospital or ED. The Clinical Care Lead will
support the Program Manager in day to day operations of the program.

Responsibilities

  • Provides information about the option to enrollee, including a description of the role of the

CPC and the enrollment process

  • Performs SDOH Assessments on all new enrollments
  • Identify areas where member may need care coordination and support.
  • Use information from the SDA and Comp assessment to build the individualized care plan of

member’s goals/needs

  • Ensures care plan uniquely represents enrollee, including a cohesive overview of the enrollee’s

strengths and challenges, interests and goals, and the accommodations provided to support enrolleedecision-making

  • Updates care plan regularly to reflect progress towards goals or to indicate changes in status
  • Assist Program Manager in daily program operationsExperience
  • Masters in related field and
  • Current LCS/LICSW, LMHC, LMFT or LADCI
  • 3 Years of Clinical Experience or Previous supervisory experience preferred
  • Minimum of three (3) years of experience providing care management in a

health and/or human services organization required

  • Previous work experience working with persons with disabilities preferred

Job Type: Full-time

Pay: $80,000.00 - $83,000.00 per year
