About the Department and Division

The Chatham County Public Health Department’s mission is to build a healthy Chatham County through community partnerships and a commitment to equity. Within the department, the Clinical and Community Health Services (CCHS) division houses a range of services and has recently transitioned out of primary care services to focus on upstream, community-based, and core public health services.

Job Summary

The Community Health and Social Work Services Supervisor position plays an important role in advancing this mission, leading change within the division, and identifying opportunities to develop effective, efficient, and sustainable services. The primary purpose of this position is to lead, support, and manage the social work and case management teams within the division. As part of this leadership role, this position provides direct supervision to 5 staff: 4 Social Worker positions, including 2 Community Alternatives Program (CAP) Case Management Social Workers and 2 Public Health Social Work Program Coordinator positions; and a Foreign Language Interpreter position.

CAP is a cost-effective home and community-based program designed to assist families in caring for medically disabled adults instead of placing them in a nursing facility. The CAP Case Management Social Workers work closely with the beneficiary, their family, caregivers, medical providers, home health agencies and medical supply companies to develop a plan of care that allows the beneficiary to remain living safely in their home.

The innovative Public Health Social Work Program seeks to improve the health and well-being of vulnerable populations in the county through the development and implementation of programs and services that address the social drivers of poor health, with content specific areas including Substance Use Prevention and Harm Reduction, Injury and Violence Prevention, Mental Health Systems of Care and Centering Equity. Services include work focused on Child Passenger Safety (car seat installation, inspection, and education) and Newborn-Postpartum Home Visits (along with a Public Health Nurse), where health assessments are provided for both mother and child, offering social support and educational resources.

The Foreign Language Interpreter is a vital piece to the Community Health and Social Work section within Clinical and Community Health Services. The interpreter knows the community, where resources are lacking - and helps coordinate bringing different community partners together for engagement opportunities. The interpreter assists with home visits, community outreach events, and completes translations for the department.

The Community Health and Social Work Services Supervisor will work closely with this wonderful team to link community members to needed resources and build systems and services to advance health in Chatham County, with a focus on vulnerable populations. This position is an excellent opportunity for an experienced public health professional with expertise in local health department community health services and compassion for diverse clients. The ideal candidate will thrive in an evolving environment that requires managing multiple projects while providing excellent internal and external customer service. The ideal candidate will also possess strong leadership, communication, and change management skills and be adept at working with internal and external partners to deliver high-quality, equitable services.

Minimum Training and Experience

Master’s degree in social work from an appropriately accredited institution and two years of directly related experience; or a bachelor’s degree in social work from an appropriately accredited institution and three years of directly related experience; or a master’s degree in a human services field from an appropriately accredited institution and three years of directly related experience; or a bachelor’s degree in a human services field from an appropriately accredited institution and four years of directly related experience; or a bachelor’s degree from an appropriately accredited institution and five years of directly related experience; or an equivalent combination of education and experience.

Salary

Grade 67: $58,975 - $75,193
