RESPONSIBILITIES:
30% 1.0* Assesses the presenting and underlying needs of identified clients by utilizing appropriate tools such as the ANSA, GAD 7, PHQ 9 and Recovery Capital Scale and collecting information from members of the treatment team and natural supports.
20% 2.0* Develops clear and measurable plans to increase the positive health outcomes of patients by collaborating with the consumer, consumer’s natural supports and member of the treatment team which promote the importance of social support and good lifestyle choices.
20% 3.0* Assists patients to minimize the negative impacts of chronic health conditions on daily living by providing the patient with education, support and technology to monitor and manage symptoms and to problem solve identified health issues.
10% 4.0* Promotes the health care of patients by assisting in building the skills necessary to independently arrange and participate in medical appointments, by linking the patient to other programs, services and agencies and by advocating on behalf of the patient with medical/service providers and natural supports.
5% 5.0* Serves as the point of contact for patient information by timely and accurately communicating information to the treatment team and individuals identified by the patient as well as ensuring that the patient is aware of information provided by the treatment team and identified individuals.
5% 6.0* Provides comprehensive transitional care services by ensuring that the needs and desires of the patient are addressed as part of a plan to transition patient care to a different provider, level of care, living environment or other case discharge.
5% 7.0* Advocates, manages and communicates mental health resources to JMH employees.
3% 8.0* Participates in community events and remains involved in local initiatives.
2% 9.0* Creates policies that promotes the betterment of mental health for JMH employees.
10.0* Conducts all activities in compliance with applicable laws, regulations, standards and hospital policies and procedures including Blood and Body Substance Precautions.

EDUCATION, EXPERIENCE AND TRAINING: Completed Bachelor's degree in Social Work, Psychology, or related behavioral health field. Must have the Indiana Community Health Worker (CHW) Certification or obtain certification within 6 months of employment. BLS is required or must be obtained within 6 months of employment. Must hold a valid driver's license with acceptable driving record and provide own, insured transportation.
