HIV Benefits Specialist (Part-Time, 30 Hours) Job at East Valley Community Health Center, Inc
We have an exciting opportunity to bring onboard a HIV Benefits Specialist to join our Pomona team!
Responsibilities:
- Conduct intakes and benefits screenings, assessments, complete service plans, and required documentation on all assigned patients in a timely manner (24-48 hours is standard).
- Provide resources and benefit referrals for patients as needs arise.
- Advocate and assist patients in obtaining all needed medical insurance benefits they are eligible for, including but not limited to, SSI, MediCal, Medicare, medical insurance through Covered California, VA benefits, and private insurance.
- Advocate and assist patients in applying for other needed benefits such as Unemployment benefits, Cal Fresh, General Relief, State Disability, etc
- Provide benefits management to patients currently enrolled in benefits services to assure that they continue to receive and keep their benefits.
- Assist patients in transitioning, from MediCal Fee for Service to new MediCal Managed Care.
- Assist patients with ADAP and OA-HIPP application and re-enrollment separate from benefit service hours. Keep up required charts and documentation for ADAP and OA-HIPP client charts as determined by CDPH.
- Provide PrEP-AP assistance as needed.
- Travel from site to site depending on client needs.
- Perform other related duties as assigned
Requirements:
- Six months to one year experience making eligibility determinations and assisting clients in accessing public benefits or public assistance programs or similar experience.
- Basic knowledge of HIV/AIDS related issues.
- Must be flexible
- Bachelor’s Degree in related field.
- Must be bilingual in English and Spanish.
- Ability to work as a team
- Computer/data entry basic skills.
- Must relate well to all cultural and ethnic groups in the community.
- Detail oriented with strong organizational skills.
