Provide in-home visits to assess patient needs and initiate appropriate interventions.

We are looking for individuals that understand the value of alleviating pain and suffering at end-of-life while meeting a shared goal of helping individuals live their final days with dignity and comfort. We currently have needs for weekend days and are open to full time and part time employment.

Registered Nurse (RN) or License Practical Nurse (LPN) with current CO license
Maintains current CPR Certification that meets American heart Association BLS Guidelines
Licensed driver with automobile that is insured in accordance with state requirements
Able to lift, position, and/or transfer patients

Essential Job Functions / Responsibilities
Provide in-home visits to assess patient needs and initiate appropriate interventions.
Work within inter-disciplinary team while collaborating with physicians regarding patient needs
Administer medications as appropriate
Teaches patient and caregivers care techniques as appropriate
Provide mental and emotional support to patients and families as appropriate
Complete accurate and relevant clinical notes in EMR
Collaborate with RN Case Manager for needed changes to patient care plan
