We will be a big FAN of yours too if, at a minimum, you have:

· A current license as a Licensed Vocational Nurse (LVN) issued by the state of California,

· Two years of relevant LVN experience working with Individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or in the care of the medically fragile,

· A current driver’s license and California automobile insurance coverage for driving your personal vehicle to and from various community locations either independently or with other team members and/or Individuals, and if a company vehicle is available for use, one must also be 21 years of age, and

· Ability to read, write and speak English. Bilingual in English/Spanish is preferred.

But here’s a known family secret, being part of one, does take effort – it’s all about what you do (your responsibilities/contributions) and how you do it (your behaviors/superpowers) -- so here are some of the things you can expect to experience as you build your FAN club in this house.

There will be challenges:

· Just like families, disabilities come in different types. You will see many of them along with the mental health challenges in the Individuals (aka the Program participants) that we serve. You will also learn about the different agencies and resources we work with to help you provide support directly, or indirectly, to the Individuals.

There will be chores:

· You will assist the Program Director and consult directly with a Registered Nurse to provide training and support to the team members who directly provide care to the Individuals with specific medical conditions at the Individual’s home. As a key member of an interdisciplinary team, you will use your superpowers to review and implement Restricted Health Care Plans and Health Maintenance Plans. You will give accurate input when evaluating needs, recommendations for enhancements or changes and reporting on the progress of Individuals to their health care provider and follow-up so the highest quality of service is maintained.

· To ensure the health and safety of all, you will provide In-service training, maintain on-going health progress notes, closely monitor medication logs, perform basic incidental treatments, if necessary and within your scope of practice. You will assist in the development and maintenance of program policies and procedures and provide general and Individual-specific training to team members as requested and train new team members in the home (e.g., Ostomy, G-tubes, Catheters, Pressure Sores, and other Restricted Health Care Plans required by Community Care Licensing). Additionally, you may be asked to assist in conducting daily health assessments and provide infection control testing, such as COVID-19 as required.

There will be chats, C.A.R.E., & cheers:

· You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!

· You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).

· You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.
