We are leading the health care system revolution going from occasional, relatively ineffective visits with providers to robust, 24 x 7 high tech/high touch interactions with providers and the health system for patients with chronic disease.
The Continuous Care Center (C3) Medical Assistant’s main duties are: 1) Supports patients in successful engagement with our technology and clinical programs; 2) Employs a variety of strategies, approaches and techniques to manage a patient's physical, environmental and psycho-social health issues; 3) Identifies and resolves barriers that hinder effective care; 4) Ensures patient is progressing towards desired outcomes by completing adherence calls and continuously monitoring patient care through remote patient monitoring alerts, assessments and/or evaluations in collaboration with the primary nurse; 5) Follows established guidelines/procedures using our digital healthcare platform.
C3 Medical Assistants perform under physician and nurse general supervision.
Reports to the Supervisor and Manager of the Continuous Care Center.
Responsibilities and Duties
Core responsibilities:
Patient Management Responsibilities:
- Manage program adherence, alerts with nurse collaboration, and administration of the program components with patients.
- Engage patients and develop trusting relationships to inspire and motivate patients to adhere to RPM program and optimize successful health outcomes.
- Evaluate patient needs and requirements to achieve and/or maintain optimal wellness using Raziel Health’s digital health platform.
- Assess Medicare, Medicaid, and Healthplan patients holistically, and provide education, support, and interventions via telephone and video visits.
- Guide patients and their caregivers toward resources appropriate for their care and well-being.
- Assess patient’s physical, environmental and psycho-social health issues in collaboration with a multi-disciplinary team, employing a variety of strategies/techniques to manage appropriately and provide timely intervention.
- Assist to identify and remove financial and functional barriers that hinder effective care.
- Escalate timely clinical assessments to nurses and/or physicians in a clear, concise manner to promote independence at home.
- Participate as a multidisciplinary team member and provide direction to other C3 team members to maintain and improve patients’ health at home.
- Identify opportunities for process improvement and optimal ways to engage patients and ensure program compliance.
- Maintain strict patient and organizational confidentiality.

Day-to-day activities
Patient Management Responsibilities:
- Identify patient compliance with 16 program readings per month and complete adherence outreach reminder calls.
- Identify patients requiring their monthly summary, review patient notes, document summary of patient’s clinical history including remote patient monitoring (RPM) readings and engage/complete telephonic monthly assessment with patient.
- Complete depression and home safety screening when relevant for patients.
- Collaborate with provider offices regarding patient needs as necessary.
- Troubleshoot equipment with patients when needed to ensure patient adherence and reading compliance.
- Responds and report user needs when necessary; providing appropriate support. IE: device installation, device replacements, device troubleshooting, and adherence coaching.
- Meet quality and productivity metrics daily.
Qualifications and Skills
Required:
Hard skills:
- Intermediate to advanced computer skills as evidenced by ability to navigate multiple systems, utilizing dual computer monitors.
- Autonomous decision making, troubleshooting and problem-solving skills related to periodic system issues.
- Proficient typing and computer navigation.
- Training and understanding of seniors is a plus.
Soft skills:
- Capacity to manage multiple or competing priorities including use of multiple computer applications simultaneously.
- Excellent communication, interpersonal and empathetic skills.
- Effective problem solving and appropriate application of clinical knowledge.
- Advanced telephonic and virtual communication skills.
- Ability to identify challenges and roadblocks and appropriately communicate them to Raziel management.
- Ability to identify with the mission of the company.
Qualifications:
- Active Certified Medical Assistant and/or training from an accredited school.
- Minimum of three (3) years of broad clinical experience.
- Demonstrated clinical knowledge and expertise as evidenced by providing intervention to manage a variety of chronic conditions.
- Must have a separate home-office to ensure absolute and continuous privacy while you work to comply with HIPAA requirements.
- Must have accessibility to high-speed DSL or cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance.
- Ability to work 40 hours per week between the hours of 8am-5pm EST with occasional Saturdays.
Preferred:
- Experience as a remote worker.
- Previous adult chronic conditions care management.
- Experience with motivational interviewing.
- Experience with MCG or CMS guidelines, assessment and documentation practice.
- Bilingual English/Spanish. Able to speak, read and write in both languages without limitations or assistance.
