We are currently seeking a Full Time RN Care Coordinator in Maine. The ideal candidate will live in the Bangor/Bucksport/Ellsworth area with ability to travel in the Maine area.

Job Summary:

In charge of residents, care plans, and assessments for all 9 Magnolia Assisted Living facilities in Maine.

Supervisory Responsibilities:

· LPN’s

Duties/Responsibilities:

· Completes assessments and care plans for all Maine residents, within regulatory guidelines

· Complete RN quarterly assessments

· Oversee the care of all Maine residents through oversight, teaching, and training staff

· Ensure resident files are prepared for survey

· Assist to complete POC (plans of correction)

· Complete service plans within established time frame for all residents

· Ensure proper and timely communication with MDS Coordinators to meet established regulations.

· Collaborative role with human resources, recruitment, retention of new staff through training and evaluation.

· Identify and report areas of improvement opportunities.

· Interfacing with social workers/hospital workers for resident placement

· Point of contact for the State

· Conduct tours, if not completed by House Manager

· Conducts new resident paperwork and transitions medical records

· Will travel to all 9 facility locations in Maine

· Allocate 20 hours a week to Marketing and 20 hours a week to assessments

Required Skills/Abilities:

· General knowledge of nursing and health care terminology

· Excellent patient care skills

· Experience working in an assisted living facility

Education and Experience:

· Post-secondary Registered Nurse Degree

· Current state license required

Physical Requirements:

· Prolonged periods of sitting at a desk and working on a computer

· May require prolonged periods of standing, moving and bending

· Must be able to lift up to 25 pounds at times

Job Type: Full-time

Pay: $70,000.00 - $75,000.00 per year
