We are looking for a client-service focused, smart, hard-working RN to join our Northern New Jersey team. This is a great position for someone who wants to do meaningful, mission-driven work, and to grow professionally.

In this role you will work with prospective and current clients, their families, and other professionals involved in their care to ensure they have a positive experience from the start of their home care services. This is done by providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person nursing assessments. You will oversee the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans.

You will provide regular, proactive communication to all parties involved in each client's care, revise care plans as often as necessary, and develop strong and trusting relationships with Tribute's clients and their families.

The role will involve weekly travel in the Northern New Jersey area to assess and visit clients and has the flexibility to work from wherever you work most effectively when not visiting or assessing clients in their homes.

The ideal candidate will bring the following:

Education & Credentials:

• Bachelors or Associates Degree in Nursing from an accredited college
• Licensed as a Registered Nurse in the state of New Jersey

Experience & Skills:

• At least two years of public health nursing experience, preferably in home care
• Progressive professional responsibilities in public health nursing
• A track record of excellence
• Unwavering, passionate commitment to the customer experience and service excellence
• Strong writing and communication skills
• Strong attention to detail and organization
• Collaborative spirit

Proof of COVID-19 vaccination or exemption based on medical or religious reasons is required.
