Bestcare is seeking multiple FULL-TIME AND PER DIEM RN FIELD SUPERVISORS at our Bronx, Manhattan, and Nassau locations. As a field supervisor, the right candidate will further Bestcare’s goal for our patients to function safely, at their optimum level of independence. Through education, communication, empowerment, and technology we are focused on transformative care while optimizing patient and employee satisfaction.

Job Responsibilities:

· Conduct initial home visits to assess patient needs and develop the Aide Plan of Care

· Communicate with a patient’s physician and other team members to ensure delivery of appropriate patient care

· Supervise patient care provided by our Home Health and Personal Care Aides

· Participate in orientation, teaching, and evaluations of care providers

· Responsible for the review and maintenance of patient records, using telehealth modalities for early intervention of patient issues

Job Qualifications:

· Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience

· Must have a valid driver’s license and transportation (other than Manhattan). Monthly Metrocards will be provided for RNs at the Manhattan location

· Previous home care experience is a plus

· Bilingual Spanish and English language abilities is a plus
