We are seeking a compassionate and motivated individual to join our Servicelink Information and Referral/Long Term Support Services team!

This individual will provide knowledge to referrals in the following areas: Community Resources (access and payment structure); Long Term Support Services for older adults and individuals with a disability; DHHS programs such as Long-Term Care Medicaid and Medicare Savings Programs, and general knowledge of Medicare.

Qualifications:

• Associates Degree in a related field; Bachelor’s Degree preferred.
• At least one-year of experience in social service related work, preferably in the areas of aging and disabilities
• Ability to navigate long-term care settings including nursing homes, assisted livings, hospital and/or rehab settings
• Proficient computer skills, Microsoft Office application suite preferred
• Obtain all mandatory certification as required within 6 (six) months and 1 (one) year.
• Strong oral and written communication, interpersonal skills and analytical and problem solving ability
• Valid driver’s license and reliable transportation

The individual in this role should also be comfortable with computers, have excellent communication skills and be willing to meet with individuals at their home or in the office. This position is based out of Nashua, NH. Additional responsibilities include:

• Performs person centered counseling and assessment of individual’s situation/needs for assistance in referrals to long-term services and support through telephone, walks-ins, email and multi-media.
• Completes eligibility screening and provides information to assist individuals in understanding and applying for State and Federal program.
• Develops care plans and complete follow-ups on received contacts for long-term care assistance.
• Assist with information and referrals on contacts received in the office.
• Assist with all aspects of the Medicare program especially during the Medicare Annual Enrollment Period.
• Completes documentation of received contacts using protocols/process as outlined with the DHHS database requirements.
• Provides long-term support counseling to select persons in hospitals, rehab facilities, and/or nursing homes.
• Attends meetings as mandated by contract or required by State.
• Obtains and maintains certification requirements as mandated under the State contract.
• Assists with general office support to include answering phones and assisting with walk-ins.

Hours: Full-time, 40 hours

Compensation: $41,600 annually.
