We provide psychiatric care to geriatric patients who require inpatient hospitalization due to mental disorders like cognitive illnesses.

As Social Worker, you will be instrumental in effectively providing quality patient care, displaying advanced skills in social assessments, treatment planning and discharge planning. The Social Worker will lead Primary Treatment Modalities, Group, Family and Individual Therapies.

These are some of the things our Social Workers do:

• Our Social Workers will keep apprised of and follow all standards for the state, our accrediting body, and the rules and regulations of the hospital.
• Complete an age specific psychosocial evaluation on assigned patients in a timely fashion.
• Engage in timely, efficient discharge planning, assuring the transition of patients to the appropriate level of care as determined by the patient's needs.
• Attend and participate in multi-disciplinary treatment staffing and in the development and revision of treatment plans.
• Assist in the orientation of new patients to the program.
• Provide for patient comfort, safety, respect, and confidentiality.
• Complete documentation requirements and assessments in a timely and professional manner.
• Assist with pre-admission screenings, as applicable.
• Ensure health and safety of all patients by observing mental and physical conditions of patients on each shift.
• Assist with intake and admissions, as needed.

What we are looking for in our Social Worker:

• Master's required, LCSW preferred.
• At least one year of experience in a behavioral health hospital is preferred, as well as experience with an older population.
