VNA Community Outreach Specialist Job at Holyoke VNA & Hospice
The Holyoke VNA & Hospice Life Care is looking for a Per Diem Community Outreach Specialist with Home Care experience required. The Community Outreach Specialist must champion and promote the mission and vision of the organization and project a positive and professional image for the Agency in the community. Enhance and promote the entire Agency, specifically the VNA, in the marketplace by regularly meeting with, communicating with and providing information and education to professional, facilities, organizations, and consumers regarding services provided as well as coverage criteria and reimbursement for these services. Collaborate with referral sources, Agency staff, physicians, and other professionals to ensure that patient services are developed and delivered in a safe, effective manner to assure that customer satisfaction is exceptional. Maintain an in-depth knowledge of the community, health care facilities, and referral sources appropriate for agency services. Assist in establishing targets and goals for development of all service lines in the market. Participate in assigned committee activities and adhere to the agency policies, which includes the Corporate Responsibility program. Communicate with customers in a competent, respectful manner to encourage positive attitudes about the agency. Assume a regular plan in the on-call schedule, assists and supports other departments when needed. Perform other duties as assigned and/or requested.
Responsibilities:
Actively perform job duties in a manner that will promote and enhance the organization's mission of providing high quality services to all of its customers. Conduct all activities in full compliance with all relevant laws and with regulatory requirements affecting business of the agency. Comply with the agency Code of Ethics, Code of Conduct and other policies as set forth by the agency. Participate in the agency process improvement plan through involvement in process improvement committees as assigned by supervisor, identifying areas for improvement in job functions and reporting these to supervisor, and performing and recording quality control tasks as assigned. To be courteous and respectful of fellow employees, patients and customers of the Agency. Establish and maintain positive interpersonal relationships that promote a willingness to work with others cooperatively and collaboratively. Adhere to agency policies and work standards on confidential information.
