Esse Health is currently seeking a full-time Care Manager - RN (Greater St. Louis and IL).

Summary:

The RN Care Manager (RN CM) is a member of the care management team and works with the Primary Care Physician to improve the health of patients. The RN CM will use the principles of nursing assessment, diagnosis, planning, implementation, and evaluation to deliver care. Essential to the role of a care manager is the ability to collaborate and participate in care coordination activities that meet the patient needs.

This RN Care Manager will provide home visits utilizing care management strategies. The RN is responsible for working collaboratively with the patient, family, physicians and the entire health care team to assess the home environment, clinical situation, health status, and barriers to care. The home health RN will perform a variety of services in the home, including physical assessments, vital signs, wound care, disease management, patient education, transition of care support, medication reconciliation and safety evaluations. The RN will also ensure that patients and families have the necessary support, resources, follow-up appointments, and home care services to improve outcomes and reduce social determinants of health (SDOH).

This position applies the standards of practice and performance in the work setting to achieve positive outcomes and ensures the performed activities are in compliance with all applicable federal and state regulations.

Preferred Qualifications:

• Licensed as a Registered Nurse in the state of Missouri and Illinois
• 5+ years clinical experience as an RN
• Home Health experience highly preferred
• Knowledge and experience working with the Medicare population, including the benefits and services available
• Training and skills in wound care assessment and treatment
• Able to work with an interdisciplinary team
• Able to work independently, manage a case load and complete assigned work on time
• Attention to detail, strong organizational and time management skills

• Proficient in utilizing electronic medical records (EMR)
