Position Summary: The Elder Advocate Assistant assists the Elder Advocate to support the independence, safety, and wellness of the Residents of St. John’s Meadows & Brickstone Independent Living. This valuable role serves as an advocate and resource to Residents by acting as a liaison between Residents, family members/caregivers, and various elder support systems throughout the St. John’s and Greater Rochester community. The Elder Advocate Assistant works to identify opportunities which promote quality of life for the Elders as well as striving to identify options to sustain the highest level of independence that is safe for each Elder. Responsibilities include:

  • Supports the Eden philosophy and St. John’s mission, vision and values.
  • Communicates with Elder Advocate regarding complex Elder situations or patterns of need in the Meadows and Brickstone communities.
  • Provides follow-up with Residents and responsible parties when incident reports are received from our Protective Services staff and supports Residents through hospital, rehabilitation, and discharge process.
  • Assist Residents, their families, and responsible parties with understanding the process for moving to a higher level of care.
  • Collaborates with Elder Advocate, Marketing team, and Administration when Residents make transitions to other levels of care.
  • Assist and support Residents through life transitions by assessing needs and identifying supportive options and resources.
  • Create, locate, and maintain lists of St. John’s specific services available to Residents as well as lists of services and resources available in the Greater Rochester area and beyond.
  • Make referrals to appropriate providers, agencies, and programs under the supervision of the Elder Advocate. Make referrals to Monroe County Adult Protective or the Mobile Crisis team as needed.
  • Assist Residents to understand and complete important paperwork and apply to applicable assistance programs and other resources.
  • Assist Residents in obtaining durable medical equipment from St. John’s Meadows’ loan closet or other community sources.
  • Collaborate with Elder Advocate to plan and facilitate speakers and presentations related to aging and/or of interest to residents.
  • Attend and participate in community chats and care conferences as required.
  • Will be required to work flexible hours.
  • Perform all other duties as assigned by management.

Qualifications:

  • An Associate's or Bachelor’s degree from an accredited program in Psychology, Social Work, Gerontology, Human Services, or related field preferred but not required.
  • Or two (2) years of experience in social work, home health care, human services, or assistance with an older adult population preferred.
  • A valid driver’s license.
  • Must possess a passion for working with seniors and improving their quality of life.
  • Responsible to uphold the safety and dignity of residents and follow the NASW Code of Ethics.
  • Actively listens to the needs of elders and core team members.
  • Excellent verbal and written communication skills, time management, and organization.
  • Computer literacy and familiarity with Microsoft Word, Outlook, and Excel.
  • Compliance with NYS Department of Health immunization requirements, including the COVID-19 full vaccine series prior to start date.
