REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES
The SDoH/Peer Specialist is part of the Behavioral Health Team and reports to the Behavioral Health Manager. There are no direct reports to this position.
POSITION SUMMARY
The primary function of the SDoH/Peer Support specialist is to support the care management team by providing education, assistance, and resources pertaining to SDOH with a focus on income, housing needs, recovery and assist in navigate insurance benefits. This position will work directly with members who are identified as having significant deficiencies in one or more SDOH as well as coordinate with community providers and supportive agencies.
ACCOUNTABILITIES
- Work with care management team, referral sources and community partners to identify members with one or more SDoH deficit
- Gather necessary information including medical records and social determinant of health status through in person or telephonic communication with members and providers
- Educate and assist members regarding eligibility for additional benefits and accessing needed services to mitigate SDoH deficits, with a focus on housing and income
Participate as a team member in transition of care activities related to income and housing needs specific to members in acute care settings or residing in nursing facilities
Organize SDoH state resources to be utilized by care management team and assist with coordinating SDoH resources for members identified as in need
Participate as a team member to collaborate in the care planning and care plan evaluation process.
Comply with all Federal, State, accreditation, and corporate regulations, standards, and policies pertaining to privacy and confidentiality.
REQUIRED QUALIFICATIONS
Education: High school diploma or equivalent.
Peer Support Specialist will model competence to their clients, through their own personal experiences, in successful self-recovery. You will act as a subject matter expert while performing a wide range of tasks to support members.
Skills: Ability to work in non-traditional settings and unstructured environments.
Excellent interpersonal, communication skills (oral and written) and organizational skills required.
Familiarity with Ohio Medicaid and income eligibility criteria and application processes preferred.
Years of Experience: At least 2 years’ experience working directly with individuals experiencing SDOH needs; experience with homelessness and financial insecurity preferred.
