CAPE ATLANTIC INK IS A NONPROFIT ORGANIZATION SEEKING TO HIRE CARE MANAGERS

Care Management is a form of case management. The Care Manager meets with assigned youth and families in the home, or the community, and is responsible for providing all the required duties of care management including facilitation of Child Family Team meetings, development, implementation and monitoring of the Individual Service Plans (ISP), coordinating services and supports (formal and informal) outlined in the ISP with the assistance of the Community Resource Administrator, and completion of the Strengths and Needs Assessment for youth and families. Care Manager transports youth and family to meetings as needed.

The Care manager completes all PE and Medicaid documentation with the family to assure health care coverage for the youth. The Care Manager must maintain accurate, timely and comprehensive records for each child as well as complete Continuous Quality Improvement Reports as determined by Quality Administrator.

The Care Manager provides on call coverage on a rotating basis so that 24/7 coverage is maintained. Care Manager upholds the policy and procedures of Cape Atlantic INK and completes all other duties as assigned. Care Manager works a flexible schedule to meet at the families' convenience.

Successful Care Managers are well-organized, detail oriented and have a strong work ethic. They are creative, empathetic, and efficient at time management.

Requirements:

Division of Medical Assistance and Heath Services (Medicaid) N.J.A.C.10:73-3.19 Staff Qualifications (sub-section c)

Care Managers must have a minimum of a Bachelor’s degree in a related field, such as social work, psychology, counseling, criminal justice or special education,and a minimum of one year (12 months)related experience working with at risk youth. Care Managers Must obtain their Care Manager Certification within 12 months of employment.

Valid Driver's license

Driving is essential to this position as well as timely and accurate documentation.

Preferred Requirements:

• Master’s degree in social work, psychology, counseling, special education, criminal justice or related social services field and a minimum of one year related experience working with at risk youth.
• Bilingual (Able to speak, read and write in Spanish and English) strongly desired
• 12 months prior experience providing home based care
• 12 months prior experience working as a Behavioral Assistant helpful
